Prepare for Your Appointment
Please remember to bring along the following to your appointment:
Mask: All patients and staff are required to wear a mask while in the building.
We will have some available if you don't have your own.
Insurance Card (if using health insurance). See the list of insurance plans
accepted at BVWHC. Plans vary even within the same insurance company.
Please call your insurance company to confirm that we are an In-Network Provider
for your specific plan and to verify your coverage.
Patient Forms: Fill out forms (below) and bring along to your visit OR arrive
15 minutes before your appointment time to fill them out.
Payment: payment is due at time of service.
Plan to Arrive on Time: Please see our Late Policy below. Should you need to cancel or reschedule your appointment, please contact our clinic no less than 24 hours prior to your scheduled appointment at 303-442-5160 or email us at email@example.com.
Please complete all forms that are indicated for your visit type and return them to firstname.lastname@example.org. Another option is to print the paperwork, complete it by hand, and bring it with you to your appointment. If you are unable to complete this paperwork before your visit, we will need you to arrive 15 minutes prior to your scheduled appointment time. If you do not arrive 15 minutes prior to your appointment, you may be asked to reschedule.
Medical Records Request Forms
To request a copy of your medical records or to request your records to be sent somewhere else (doctor, insurance company), download the following forms, fill them out, and return to Women’s Health. Please have your ID ready when you drop off these forms. To request your records without coming into the clinic, please visit the patient portal.
No Show/Same Day Cancellations/ Late Policy
In order to provide excellent care to each patient in a timely manner, we have implemented a “no-show/same day cancellation/late arrival” policy as of November 16, 2020.
In the event you are unable to make it to your scheduled appointment and need to cancel or reschedule, please contact our clinic no less than 24 hours prior to your scheduled appointment at 303-442-5160 or email us at email@example.com. Patients with two or more occurrences (no shows or same-day cancellations) may be subject to suspension from BVWHC for a period of three months.
If you are a new patient to our clinic, please complete the Patient Forms on our website (above) or come in 15 minutes prior to your appointment time to complete paperwork. If you do not have your paperwork completed and do not arrive 15 minutes before your appointment time, we may have to reschedule your appointment.
If you are an established patient at our clinic (seen within the last 3 years) and you arrive more than 5 minutes after your scheduled appointment time, we may have to reschedule your appointment.
